By using VevaRx’s services, you acknowledge and agree to the following regarding telehealth
care:

1. Nature of Telehealth Services
VevaRx facilitates access to telehealth services provided by licensed healthcare providers.Telehealth involves the use of electronic communications and digital technologies to enable healthcare services when the provider and patient are not in the same physical location.

VevaRx utilizes asynchronous (store-and-forward) telehealth modalities. This means you will submit your health information (including medical history, forms, and questionnaires) for review by a licensed provider at a later time, rather than interacting in real time.

2. Provider-Patient Relationship

A patient-provider relationship may be established through asynchronous review of your submitted information, where permitted by applicable law. The provider will determine whether sufficient information is available to make a clinical decision or whether additional information, interaction, or referral is necessary.

3. Limitations of Telehealth

Telehealth services have inherent limitations, including but not limited to:
● The absence of a physical, in-person examination
● Potential for incomplete or inaccurate information if patient-provided data is incorrect or incomplete
● Possible delays in communication due to the asynchronous nature of the service In some cases, telehealth may not be appropriate, and you may be advised to seek in-person care.


4. No Guarantee of Treatment or Prescription


Submission of information through VevaRx does not guarantee that you will receive a diagnosis,treatment, or prescription. All clinical decisions are made at the sole discretion of the licensed provider based on medical necessity and applicable standards of care.


5. Privacy and Security


VevaRx uses reasonable administrative, technical, and physical safeguards to protect your personal health information in accordance with applicable privacy laws. However, no electronic Transmission is entirely secure, and there is a risk of unauthorized access.


6. Pharmacy and Fulfillment


VevaRx is not a pharmacy and does not dispense or fulfill medications. If a prescription is issued, it will be transmitted to a licensed pharmacy, which is solely responsible for verification, preparation, and dispensing.


7. Patient Responsibilities


You agree to:
● Provide accurate, complete, and truthful health information
● Update your information as necessary
● Follow provider recommendations and seek in-person care when advised


8. Emergencies


Telehealth services are not appropriate for medical emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room.


9. Consent to Telehealth


By using VevaRx, you voluntarily consent to receive healthcare services via asynchronous telehealth technologies and acknowledge that you have read and understand this informed consent.


10. Right to Withdraw


You may withdraw your consent to telehealth services at any time by discontinuing use of the platform; however, doing so will not affect any care already provided.
